Thenar Eminence Myofascial Release
Self-massage and release of the thenar eminence muscles (base of the thumb), frequently tense in patients with carpal tunnel syndrome. The release reduces pressure on the median nerve at the entrance to the carpal tunnel and improves thumb opposition.
How to perform
- Starting position. Rest the affected hand on a table or in your lap, with the palm facing up.
- Step 2. With the thumb of the contralateral hand, firmly press the fleshy base below the thumb (thenar eminence).
- Step 3. Make slow circular movements for 30 seconds, searching for painful trigger points.
- Step 4. When you find a tender point, hold sustained pressure for 30–60 seconds until the pain reduces — this is the ischemia-hyperemia effect.
- Return. Finish with active thumb opposition movements (touch each fingertip with the thumb tip) 10 times.
When not to perform
- Thumb-base osteoarthritis (rhizarthrosis) in an acute painful phase
- De Quervain's tendinopathy in an acute phase
- Local hand infection
- Distal median nerve neuropathic pain with intense paresthesia
- Recent hand surgery
- Recent fracture of the first metacarpal
