Avoid the great saphenous vein.
Location (classical)
On the medial aspect of the ankle, in the depression inferior and slightly anterior to the medial malleolus, at the junction of straight lines drawn along the anterior and inferior borders of the medial malleolus (midway between the medial malleolus and the navicular tuberosity).
How to find it (practical)
Seated with the feet supported or supine, locate the point at the intersection of the lines passing along the anterior and inferior margins of the medial malleolus.
Anatomy
Skin, subcutaneous tissue, medial dorsal cutaneous nerve from the superficial fibular nerve, branches of the great saphenous vein, crural fascia, medial crural cutaneous branches of the saphenous nerve, inferior extensor retinaculum, between the tendons of the tibialis anterior and the extensor hallucis longus, medial anterior malleolar artery, branches of the deep fibular nerve, and the talus.
Needling
0.3–0.6 cun perpendicular, oblique, or transverse (subcutaneous) deep to the tendons toward ST-41.
Etymology
Shang (商), one of the five notes of the ancient five-tone musical scale, represented by Metal; Qiu (丘), mound. The point lies below the medial malleolus, in a place that resembles a mound, and belongs to the Metal element.
