Thumb Opposition Training
Specific motor training of thumb opposition — a complex movement involving simultaneous flexion, abduction, and rotation. Essential in De Quervain's tenosynovitis rehabilitation and in any condition that compromises fine hand dexterity.
How to perform
- Starting position. Rest the hand on a table with the palm facing up.
- Step 2. Touch the tip of the thumb to the tip of the index finger, forming a perfect O — hold for 1 second.
- Step 3. Next, touch the tip of the thumb to the tip of the middle finger, then ring, then little finger, always forming an O.
- Step 4. Return by touching in reverse sequence: little, ring, middle, index.
- Return. Perform the movement slowly and deliberately — the precision of the touch matters more than speed. For progression, perform with eyes closed, training proprioception.
When not to perform
- Recent median nerve surgery
- De Quervain's tenosynovitis in an acute inflammatory phase
- Rhizarthrosis in a painful flare
- Active rheumatoid arthritis of the interphalangeal joints
- Carpal tunnel syndrome with motor paresis
- Recent finger fractures

Related Exercises

Hand and Wrist Range of Motion
A sequence of active hand and wrist movements to maintain range of motion in rheumatoid arthritis. Focus on flexibility without joint overload.

Flexor Tendon Gliding
A sequence of hand positions that selectively mobilizes the flexor tendons through the carpal tunnel. Improves tendon glide and reduces adhesions.

Thenar Eminence Myofascial Release
Self-massage and release of the thenar eminence muscles (base of the thumb), frequently tense in patients with carpal tunnel syndrome. The release reduces pressure on the median nerve at the entrance to the carpal tunnel and improves thumb opposition.
