Deep perpendicular insertion carries a high risk of pneumothorax. Caution: perpendicular insertion, especially in lean patients, carries substantial risk of inducing pneumothorax.
📍 Location (classical)
On the lateral aspect of the chest on the midaxillary line, 6 cun below the axilla, in the seventh (or sixth) intercostal space.
🧭 How to find it (practical)
In lateral decubitus, with the arm raised over the head, locate the point in the sixth intercostal space, on the midaxillary line, 6 cun below the axillary fossa.
🔬 Anatomy
Serratus anterior and latissimus dorsi muscles. Innervated by the long thoracic nerve and the 6th intercostal nerve.
💉 Needling
Transverse-oblique insertion along the intercostal space, 0.5 to 1 cun.
📜 Etymology
Da (大), great, encompassing; Bao (包), content. The point belongs to the Great Luo (connection) of the Spleen-Pancreas Meridian, Tai Yin of the Foot, and governs the Yin and Yang Meridians. Pi (Spleen) nourishes the five Zang Fu and the four limbs.
