Precautions
HIGH RISK. Strict adherence to the angle of insertion is mandatory. Deep perpendicular insertion will perforate the trachea. Deep inferior insertion at an incorrect angle may perforate the aorta or the brachiocephalic artery.
Location (classical)
On the anterior midline, in the center of the suprasternal fossa.
Anatomy
Sternohyoid and sternothyroid muscles. The trachea lies directly posterior. The aortic arch and the brachiocephalic trunk are deep in relation to the sternum.
Needling
COMPLEX INSERTION. First, insert perpendicularly 0.2 cun. Then, direct the needle inferiorly along the posterior surface of the sternum (anterior to the trachea) for 0.5–1.0 cun.
Etymology
Tian (天), heaven; Tu (突), chimney. The point is located in the upper region of the trachea, like a chimney for the Qi of the lung.
