Perpendicular insertion
Precautions
Deep needling will directly contact the sciatic nerve (causing a strong electrical sensation in the leg). Ensure the sensation is not excessively painful.
Location (classical)
On the posterolateral aspect of the hip, at the junction of the lateral one-third and the medial two-thirds of the line connecting the most prominent point of the greater trochanter and the sacral hiatus.
How to find it (practical)
In lateral decubitus, locate the point in the posterior depression of the greater
trochanter, at the junction of the lateral one-third with the middle one-third of the distance between the greater
trochanter of the femur and the intergluteal cleft. (Fig. 11.21)
Anatomy
Gluteus maximus and piriformis muscles. The thickest part of the sciatic nerve passes directly deep to this point. Innervated by the inferior gluteal nerve.
Needling
Perpendicular insertion directed slightly inferiorly, 1.5–3.0 cun.
Etymology
Huan (环), a ring; Tiao (跳), to jump. The point is located at the hip joint, which is responsible for the act of jumping.
