Precautions
High risk of pneumothorax. Deep perpendicular insertion is strictly contraindicated. Caution: risk of penetrating the Heart or causing pneumothorax.
Location (classical)
On the chest, in the fourth intercostal space, 5 cun lateral to the anterior midline (approximately 1 cun lateral to the nipple).
How to find it (practical)
In dorsal decubitus, locate the point in the intercostal space of the 4th rib, 1 cun lateral to the nipple.
Anatomy
Skin, subcutaneous tissue, mammary and lymphatic tissues, lateral cutaneous branches of the intercostal artery and nerve, pectoralis major and pectoralis minor muscles, internal thoracic nerve, long thoracic nerve, and lateral thoracic artery.
Needling
0.5–0.8 cun obliquely or transversely (subcutaneous) along the path of the intercostal space, in the same direction as or against the channel pathway.
Etymology
Tian (天), high (heaven); Chi (池), pool (pond). The point lies lateral to the breast, as if the milk produced there originated from a celestial reservoir.
