High risk of pneumothorax. Deep perpendicular or anterior insertion is contraindicated.
Location (classical)
In the supraspinous region, in the depression midway between GB-21 and SI-13, over the superior angle of the scapula.
How to find it (practical)
With the shoulder relaxed, locate the point at the superomedial angle of the scapula.
Anatomy
Trapezius and levator scapulae muscles. Innervated by the accessory nerve and the dorsal scapular nerve.
Needling
Perpendicular or oblique insertion directed inferiorly, 0.5–1.0 cun.
Etymology
Tian (天), heaven, upper part; Liao (髎), cleft, foramen. The point lies in a foramen above the scapula.
