Precautions
Deep thoracic needling may reach the pleura and cause pneumothorax. In the lumbar region the relationship to vessels and nerve branches varies with level and trajectory. The textbook range does not establish a safe depth for every individual.
Location (classical)
From T1 to L5, one pair per level at the inferior border of each spinous process, 0.5 cun on either side of the posterior midline. There are 17 pairs, or 34 points.
How to find it (practical)
Identify the spinous processes from T1 to L5. At each level, measure 0.5 cun laterally from the midline.
Anatomy
In the paraspinal musculature, the relationship to the posterior rami of the spinal nerves and accompanying vessels varies by vertebral level.
Atlas differences
The standardized series has 17 thoracic and lumbar pairs. Some schools describe cervical extensions separately. Zhenjiuxue gives the same range as the team’s textbook. Focks distinguishes thoracic and lumbar approaches and describes a medially oblique variant. An anatomical study of the lumbar region does not establish a single 45-degree angle for every level.
Needling
For the 17 pairs of Jiaji points from T1 to L5, the textbook describes perpendicular needling at 0.3 to 0.5 cun. It also describes bloodletting by tapping with a plum-blossom needle as an alternative and permits moxibustion.
